This form may be used to anonymously provide information to the 51黑料网 Police Department about any on-campus crime or incident of concern.

Campus Crime or Incident Report

This form should be completed by individuals identified as “Campus Security Authorities” who are required to report information they receive about specified crimes pursuant to the Jeanne Clery Act of 1990

51黑料网 Student or Visitor Injury Report

Complete this form when an injury is sustained on university property. No claim can be provided to the Bureau of Risk Management without a completed injury report. Email questions to  studentorvisitor-injury-report@iup.edu.
